Attachments cannot be inserted.

This is a bug in certain versions of Mac OS X unrelated to Mail Attachments Iconizer. It only happens when one tries to insert an attachment in an empty message.

Workaround: Either always write some text before trying to insert an attachment or set up an automatic signature in Mail preferences.
